Job Summary and Qualifications
POSITION SUMMARY:
The Case Manager serves as a liaison between medical staff, administration, utilization review, nursing, finance, medical records and other ancillary departments to ensure that patient care needs are met and treatment plans are executed. Case Manager facilitates:
- Appropriate level of care
- Appropriate setting of care
- Efficient, effective, high quality service delivery
- Timely discharge with appropriate community-based support
- Coordination of clinical, financial, and psychosocial needs
ESSENTIAL FUNCTIONS:
- Assists in the orientation of the Rehab patient to his/her individual program.
- Assumes responsibility for making sure that the individual rehab plan is implemented.
- Enables the individual plan of the patient to proceed in an orderly, purposeful, and goal directed manner.
- Promotes the program’s responsiveness to the strengths, abilities needs and preferences of the patient served.
- Promotes the participation of the patient on an ongoing basis in discussions of plans, goals and status.
- Participates consistently in team conferences concerning the patient served.
- Facilitates discharge and arrangements for follow-up and appropriate services, identifies and utilizes appropriate community resources to meet patient discharge and continuity of care needs.
- Communicates information regarding progress to appropriate persons. Communicates both verbally and in writing with the multi disciplinary team, regarding social financial or discharge needs of the patient and/or family.
- Appropriately identifies difficult/problematic cases for discussion with the Director at discharge planning meetings or referral to hospital utilization committee.
- Participates in department quality improvement activities by identifying improvement needs, collecting data, and implementing plan.
- Demonstrates Customer Service/Service Plus principles and behaviors appropriately.
- Collaborates with physicians and health care team in planning the care of patients and their families within the accurate care setting and along the health continuum.
- Coordinates and corresponds to appropriate agencies regarding preadmission screening, concurrent, outpatient, and retrospective review process as it relates to federal and commercial insurance patients to maximize financial outcomes.
- Other duties as assigned.
REQUIRED LICENSURE/CERTIFICATIONS:
- Current RN licensure in Texas or Compact State and BCLS certified
REQUIRED EDUCATION:
- Minimum Associate degree in Nursing
PREFERRED EDUCATION, LICENSURE, CERTIFICATION, EXPERIENCE, SKILLS, KNOWLEDGE AND ABILITIES:
- Two–three years experience in an acute care setting with experience in discharge planning, and case management.
- Reads and writes using Basic English Grammar; bilingual skills preferred.
- Ability to listen, empathize and respond to patient, family, internal and external team member needs.
- Basic computer skills.
- Proficient in collecting and retrieving data from medical records and other information sources.
- Effectively applied data retrieved from the medical record to the review process.
